Not sure this counts, but adobe had no org wide tooling afaik. Each team could pick its CI, code review, repo , issue tracker etc. I was on a smaller product but it was chaos. I especially felt it when we had some CI issues and no expertise to help. It was a lot of time out the window trying to get the basics working.
