I thrive in small, high performance teams at companies of 30 people or less. Currently a top performer and I'd like to keep that up.

Right on, yea, I was here in the 90's and early 2000s. Worked at Nicolet Biomedical, then a small software company (Wingra Technologies), then joined a startup custom coding shop as a partner from 98-2006.
In 2004 I moved to the Toronto area, just came back here a couple years ago (post covid).
