And on top of it, a major source of mortality - smoking - is widely present in Europe and nearly absent in America. Drug addiction is also not a thing for the rich. The rich have no healthcare issues - in America if you can pay, healthcare is better than in Europe, and there are much fewer delays too, only problem is the price.
Just one tiny opinion and maybe a tiny piece of the puzzle is that the US allow chemicals in foods that are banned in the EU. There is likely a lot more to it but I suspect that is at least one factor.
So what it is after all? Maybe simply race?
